Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy applies to “consumer health data” that Healthful Stories, Inc. (“we,” “us,” or “Healthful Stories”) collects, uses, shares, and otherwise processes through HealthfulHeroes.com and related tools. It is provided in addition to our general Privacy Policy and addresses only the categories of information and rights described below. “Consumer health data” means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer’s past, present, or future physical or mental health status.
Categories of Consumer Health Data We Collect, and Why
We collect the following categories of consumer health data that you voluntarily choose to share:
- Health conditions, diagnoses, and symptoms you describe
- Treatment, medication, and care experiences
- Caregiving experiences relating to another person’s health
- Information that could identify you as seeking or having received health care or services
- Audio, video, images, transcripts, and written statements in which you discuss your health
- Other health-related information you enter into our surveys, forms, interviews, or profile
We collect and use this data to: create and publish patient stories for advocacy, awareness, and education; conduct and support research and scientific publication; inform health policy and legislative advocacy; operate and improve our services; and the additional purposes you have separately consented to.
Categories of Sources
We collect consumer health data from the following categories of sources:
- Directly from you, through interviews, surveys, forms, uploads, and your contributor profile
- From you through third-party sign-in or account tools you choose to use (for example, Google or Facebook sign-in)
Categories of Consumer Health Data We Share
We may share the following categories of consumer health data:
- The health conditions, diagnoses, symptoms, and treatment or caregiving experiences you describe
- Audio, video, images, transcripts, and written statements in which you discuss your health
- Identifying information you choose to include (such as your name, likeness, and voice) when it accompanies the health information above
Third Parties and Affiliates With Whom We Share
We may share consumer health data with the following categories of third parties:
- Service providers and processors that host, store, edit, or help operate our services
- Advocacy, nonprofit, and policy organizations, and policymakers and governmental bodies
- Researchers, academic institutions, and publishers
- Media organizations and promotional partners
- The general public, where you have chosen to make your story public
Specific affiliates with whom we share consumer health data:
- Healthful Data, Inc., and any named sponsors. MHMDA requires naming specific affiliates, not just categories.
Your Rights
You have the right to:
- Confirm and access whether we collect, share, or sell your consumer health data, and to access that data, including a list of all third parties and affiliates with whom we have shared or sold it, together with an active email address or other online means to contact them.
- Withdraw consent to our collection and sharing of your consumer health data.
- Delete your consumer health data. Upon your request, we will delete it from our records, including archived and backup systems, and will notify any affiliates, processors, contractors, and other third parties with whom we have shared it to delete it as well.
- Appeal a denial of any of these requests, and to be free from discrimination or a penalty for exercising any of these rights.
We will not unlawfully discriminate against you for exercising any of these rights.

For Residents of NV, CT, and NY
Nevada Residents (SB 370)
If you are a Nevada consumer, the following applies to the consumer health data we collect from you, in addition to the disclosures above. Nevada’s consumer health data law largely mirrors Washington’s.
- We obtain your affirmative, opt-in consent before collecting or sharing your consumer health data, except where collection or sharing is necessary to provide a product or service you have requested.
- We obtain a separate, distinct consent before sharing your consumer health data.
- We will not sell your consumer health data without your separate, written authorization.
- You have the right to confirm whether we collect, share, or sell your consumer health data; to access it, including a list of the third parties and affiliates with whom we have shared or sold it; and to request deletion of your consumer health data.
- We do not collect consumer health data through a geofence around any entity that provides in-person health care services in order to identify, track, or send messages to you, or to collect your consumer health data.
- To exercise these rights, contact us at contact@healthfulheroes.com. Nevada’s law is enforced by the Nevada Attorney General.
Connecticut Residents (CTDPA — Consumer Health Data Provisions)
If you are a Connecticut consumer, the following applies to your consumer health data, in addition to the disclosures above.
- We obtain your opt-in consent before processing (including collecting or sharing) your consumer health data. This consent is not bundled with unrelated terms or other consents.
- You have the right to access, correct, and delete personal data we process about you; to obtain a portable copy; to opt out of the sale of personal data and of targeted advertising and certain profiling; and to appeal a denial of a request. To exercise these rights, contact us at contact@healthfulheroes.com.
- We do not collect consumer health data through a geofence (within 1,750 feet) around any mental health facility or reproductive or sexual health facility to identify, track, or send messages to you.
- Connecticut’s law is enforced by the Connecticut Attorney General. [Confirm current CTDPA consumer-health-data rights and any appeal/response-time specifics with counsel.]
New York Residents
If you are a New York consumer, the following applies to your consumer health data, in addition to the disclosures above.
- We obtain your authorization or valid consent before processing your consumer health data for purposes other than those strictly necessary to provide a product or service you have requested, and before selling it.
- You have the right to access and to request deletion of your consumer health data, and to withdraw consent.
